How do I adjust the size of a cardigan pattern?

There's a moss stitch baby cardigan that I want to make for my little one (it's in the one skein wonders for babies book), but the pattern is written for size 6-12 months and I'm at a total loss on how to adjust the size of a sweater. I need to size it down to a 0-3.
It's done in pieces (back, right front, left front, sleeves). As written, the back is coming out about 11 inches wide for me (50 St per row, gauge is 18 st across 4 inches). Help?
